1. Review scope
Record the practice name, website, location, services, agreed sources, reviewer, and review date. State what was excluded and what access was unavailable.
2. Search test record
For each prompt, record exact wording, date and time, search mode, relevant location context, mentions, citations, and a saved answer or screenshot where permitted. Repeat tests receive separate entries.
Example prompt to adapt: “Find a family dentist in [city] accepting new patients.” This is an example question, not verified search-volume data or a tested result.
3. Evidence and findings
| Check | Evidence to collect | Template status |
|---|---|---|
| ChatGPT Search results | Exact prompt, settings, date, answer, and citations | Not tested |
| Search crawler access | Robots rules, response headers, and hosting logs when available | Unverified |
| Website information | Public URLs showing services, location, contact details, and appointment options | Not reviewed |
| Selected local listings | Source links and any verified inconsistencies | Not reviewed |
| Referral traffic and inquiries | Consented analytics access and agreed attribution method | No data supplied |
4. Prioritized recommendations
Each recommendation should include its evidence, expected purpose, priority, owner, dependencies, and follow-up check. Label hypotheses separately. Do not estimate missed patients or revenue without supporting data.
5. Follow-up measurement
Repeat the agreed questions under documented conditions. Compare observations and, where available, referral traffic and inquiries. Report changes alongside limitations; do not attribute every change to the work performed.
